I came across this image today. To put it into perspective it was on a site that is selling a pike lure. This lure is very similar to a pike in it's body and paint job. I assume they do not have musky in their area because this lure is 12 inches long.
Anyways this picture was part of the sell job for this lure.
Is this picture real or fake?

I would say fake. If in fact that fish was attacking the smaller fish at the surface of the water it would have been moving and there doesn't seem to be any water movement from the big fish. Fake is My vote

Looks real to me. Why fake a picture with two little pike? Likely to happen in some of the back lakes I fish...
Could even be a screenshot from a good quality video.
This year I spent most of my money on beer, women and fishing...the rest I wasted.

I don't see a reason to take the time and effort to fake this image. There are enough real pics out there of more incredible pike action and situations that make the one in question to be kind of pedestrian by comparison.
So... unless the picture looks like this:
It's probably real.
